What is a Support Coordination Agency?
A Support Coordination Agency helps you get the services you need. A Support Coordinator will assist through the process of self-direction and empowerment. The Support Coordinator will provide information about the range of services available during the planning meeting.
A&S Support Services DDD Support Coordination Program
Step1: if you are registered with DDD and a resident of any of the Counties served by A&S:
A&S Support Services’s Support Coordinators will:
- Assist in identifying and utilizing community-based programs, resources, and services to develop a plan to meet your individual needs.
Step 2: Through developing the Individualized Service Plan:
A&S’s Support Coordinators will:
- Assist in identifying goals and needs in accessing community-based programs and services.
- Explore the natural supports of family, neighbors and community to assist in developing and designing a plan to meet your individual needs.
Step 3: Once your plan is approved by DDD:
A&S’s Support Coordinators will:
- Continue to be there every step of the way. Ongoing contact will ensure that necessary services are in place not for today but also for the future.

Eligibility for DDD Services:
- Be a resident of New Jersey
- Be 21 or older for services
- DDD and Medicaid eligible
